Family Medical Leave
The Town of Grand Chute complies with State and Federal laws concerning Family Medical Leave (FML). If your leave qualifies for FML under both Federal and State laws, leave used will count against your job-protected leave entitlement, Federal and State leave will run concurrently based on a calendar year.

Employees who believe they may qualify for unpaid Family Medical Leave are encouraged to complete the FML Employee Request Form and submit the completed form to Human Resources. To review Family and Medical leave under the Town's Handbook Policy, click here.

The Town promotes health and wellness of its employees by offering health insurance to all eligible employees, promotes personal health by sponsoring on-site wellness events during the year, and provides an Employee Assistance Program to every employee and member in their household.

Wisconsin Retirement System (WRS) - The WRS is a pension plan that is intended to provide eligible employees with a lifetime retirement payment (annuity) once they are vested and have reached minimum retirement age. WRS benefits are calculated using two methods, the formula method and the money purchase method. ETF calculates employee benefits using both methods and automatically pays you the higher amount.
Wisconsin Deferred Compensation (WDC) - Deferred compensation programs allow eligible employees to save and invest before-tax and after-tax (Roth) dollars through voluntary paycheck contributions, supplementing any existing retirement/pension benefits. Qualifying Event
If you got married, divorced, birth or adoption, death, change in either your spouse's or your employment status, then you have 30 days from the qualifying event to submit the Qualifying Event form with supporting documentation and enrollment forms (if applicable) to Human Resources.
Beneficiary Change Forms
Life happens! A properly completed beneficiary designation ensures that your benefits go to whom you intended to receive them at the time of your death.
The Hartford Beneficiary Form for life insurance - Please return the form to the Human Resources Office.
The ETF Beneficiary Form for your WRS - Please return the completed form directly to ETF, instructions are in the upper left corner of their form.
Change of Address
Return the completed Change of Address form to Human Resources or Payroll.
